ACT

Assertive Community Treatment (ACT) is a mobile team approach designed to help adults with mental illness.

Our confidential services can assist you and your loved ones with issues such as:

  • Symptoms that are hard to manage or do not go away
  • Difficulties meeting basic needs, protecting themselves, keeping safe, maintaining adequate housing, or staying employed
  • Often spending a lot of time in hospitals or being homeless because of symptoms
  • Legal problems due to mental illness and co-occurring disorders

A group of practitioners, psychiatrists, nurses, employment specialists, and substance abuse specialists

Continuous, Personalized Care

Various team members with small case loads ensure the best possible care. Support is provided frequently and daily if needed. Staff adjust their schedules to be flexible to client needs.

Support Where It's Needed

The majority of contacts are within the community setting. ACT is there for you as long as it is medically necessary.
